Last week, Governor Newsom announced that all teachers and school staff must be fully vaccinated for COVID-19, or be tested weekly. As a result, Parlier Unified School District will be requiring all employees to verify their vaccination status by August 31, 2021. Employees will be required to fill out the attached form below (this will also on be available on the PUSD Website- under the Resources tab; COVID 19- For Staff link) and return that form to the Human Resources Department. Certificated staff will return the form to Claudia Villegas; Classified staff will turn in forms to Emilia Alvarez.
Parlier Unified Employee Vaccination Verification Form
Employees will need to answer the following question-
I attest that (check only one box):
☐ I have been fully vaccinated (Date of final dose: ______________)
☐ I have not been fully vaccinated
☐ I decline to state.
By checking box 2 or 3, employee will be acknowledging that they are obligated to comply with the CDPH and State of California requirements to get tested weekly for COVID-19 through PUSD provided testing
This form is due to Human Resources by August 31, 2021.
Employee Health Screening:
As a result of the form above, employees will not need to state vaccination status on the Parent Square COVID Screening Questionnaire form. However, your vaccination status may impact your quarantine timeline. Please make sure you log in and complete the questionnaire before you arrive each day at school.
